Here’s how it works.

1. Enter your email
2. You’ll receive a secure link to that email to access Request Manager.

   (no username and password to remember – whew!)

3. Follow the step-by-step instructions on the screen to submit your request.
4. We will process your request, and you will receive an email with a link to download your records once the request is complete.

Please note: Standard medical record requests generally take about 2-3 business days. Requests for FMLA and/or disability forms take an average of 7 business days because those require more detailed information and approvals from your physician.
